چکیده

748 March 2013 U ncuffed tracheal tubes (TTs) have been the standard for tracheal intubation in infants and children for decades until the recent introduction of the Microcuff (Kimberley-clark, Roswell, GA) cuffed TT. evidence suggests that the incidence of perioperative airway events after use of these new TTs is small,1,2 although the evidence for their safe use in neonates and young infants is scant. We identified three infants, two of whom were born premature and one who was syndromic, who developed postextubation stridor after the use of Microcuff® TTs. Because postextubation stridor after surgery has been infrequent in infants in our neonatal intensive care unit (nIcu), we believe that this cluster of three infants with postextubation stridor may signify an underrecognized risk when these TTs are used in young infants. Accordingly, we prepared this communication, summarizing the perioperative course of the three affected infants.

Cuffed versus uncuffed endotracheal tubes in pediatric anesthesia: the debate should finally end.

500 March 2013 I this issue of Anesthesiology, sathyamoorthy et al.1 describe three cases of postextubation airway swelling in young infants. The cases are noteworthy because the endotracheal tube used in all three infants was a recently developed cuffed endotracheal tube called the Microcuff® (Kimberly-Clark, Roswell, gA), which has been specifically designed for use in pediatric anesthesia. i...

Clinical evaluation of cuff and tube tip position in a newly designed paediatric preformed oral cuffed tracheal tube.

BACKGROUND To assess the adequacy of the position of the tracheal tube cuff and tracheal tube tip in the recently introduced preformed oral Microcuff paediatric endotracheal tube (PET) using the manufacturers recommendations for Microcuff tracheal tube size selection. METHODS With Hospital Ethics Committee approval and informed parental consent, the tracheas of children from birth to adolesce...

PAEDIATRICS Prospective randomized controlled multi-centre trial of cuffed or uncuffed endotracheal tubes in small children

Methods. Patients aged from birth to 5 yr requiring general anaesthesia with TT intubation were included in 24 European paediatric anaesthesia centres. Patients were prospectively randomized into a cuffed TT group (Microcuff PET) and an uncuffed TT group (Mallinckrodt, Portex, Rüsch, Sheridan). Endpoints were incidence of post-extubation stridor and the number of TT exchanges to find an appropr...

Prospective randomized controlled multi-centre trial of cuffed or uncuffed endotracheal tubes in small children.

BACKGROUND The use of cuffed tracheal tubes (TTs) in small children is still controversial. The aim of this study was to compare post-extubation morbidity and TT exchange rates when using cuffed vs uncuffed tubes in small children. METHODS Patients aged from birth to 5 yr requiring general anaesthesia with TT intubation were included in 24 European paediatric anaesthesia centres. Patients wer...

Obstructive fibrinous tracheal pseudomembrane: a rare condition in postextubation stridor.

Obstructive fibrinous tracheal pseudomembrane (OFTP) is an airway complication that occurs after endotracheal intubation. It originates from superficial mucosal abrasion and desquamation of necrotic tracheal epithelium at the site of cuff pressure. This condition is a rare cause of postextubation stridor. We present a case of postextubation stridor secondary to OFTP, resulting in clinical featu...
